Metabolic Surgery Achieves Better Glycemic Control and Reduced Cardiovascular Risk in Patients with Type 2 Diabetes and Obesity

Bariatric surgery for obesity promotes significant and sustained weight loss, and improves glycemic control and long-term outcomes in patients with type 2 diabetes. Given the magnitude and speed of improvement in hyperglycemia and glucose homeostasis, these procedures have even been proposed for the treatment of type 2 diabetes in individuals without severe obesity, so the preferred term today is metabolic surgery.
According to evidence from the 2025 ADA Standards of Care, including data from large cohort studies and randomized controlled (unblinded) clinical trials, metabolic surgery achieves better glycemic control and reduced cardiovascular risk in patients with type 2 diabetes and obesity compared with non-surgical interventions.
In addition to improving blood glucose, metabolic surgery also reduces the incidence of microvascular disease, enhances quality of life, lowers cancer risk, improves cardiovascular disease risk factors and long-term cardiovascular events, and reduces all-cause mortality. Cohort studies matching surgical and non-surgical patients strongly indicate that metabolic surgery reduces all-cause mortality.
